The "FORM" is set up to work with a computer resident Email Handler(OutLook)... not an Internet Email Handler. So after you have completed the Form click on the 'Submit by Email' box in the upper right-hand corner of the form. The KTI-LoH-Form.xml Email browser window should open and you need to just send the Email off by clicking on the "send button" (usually upper left corner), which returns you to the filled out Form page that you can close if the form was sent. If none of the just mentioned actions take place (using Internet Email Handler), just click on the save icon and save the PDF Form ("KTI-LoH-Form.pdf") to your Computer Desktop then open up your Email Handler and attach the file to an Email sent to triple7center@comcast.net ...Thank you for your help and patience.
